WCBS will soon be releasing a new transfusion education poster campaign titled, ‘Don’t Dismiss A Near Miss’ to promote the reporting of every incident that could have resulted in transfusion of an incorrect blood component.

The reporting of near miss events is encouraged to identify and control risks before actual harm occurs, providing opportunities to improve transfusion safety and focus more on the system than on the human error.

The South African Haemovigilance Report indicates that during 2023 a total of 13 near miss events were reported nationally. The types of near miss events included ‘wrong blood in tube’ (WBIT), errors in crossmatching, mislabelling of dispatching bags, and blood bank crossmatch errors.
